New Provider Application Process

Application review and approval can take up to 90 business days. Our Executive Director will reach out to you within 2-5 business days to begin the process.

Therapy providers who wish to partner with Family Hope Foundation must meet our provider guidelines, submit all required documents, and be approved by our Executive Director.

Applications not reviewed by the start of the scholarship cycle will be available for selection by applicants during the next scholarship cycle pending approval.
